Main tasks include:
Care Navigator – Trauma
The Community Case Manager is responsible for working directly with the Hospital Responder in meeting all violently injured patients who have consented to receive services through the Hospital Violence Intervention Program. The Community Case Manager will work with the patient to engage in services both before and after the patient discharges from the hospital. The position requires the ability to engage with individuals in a trauma-informed manner, and to model effective ways of engaging with resources to support healing. The Community Case Manager will be expected to perform a significant amount of community- and home-based work and engage within a diverse range of neighborhoods.

Qualifications:
Qualifications
High school diploma required, Bachelor's degree preferred
2 years of case management experience
Experience working in violence prevention/intervention preferred
DC Peace Academy Training preferred
Effective written, verbal and interpersonal communication skills
Ability to make quality, independent decisions
Ability to maintain confidentiality
Ability to adapt to multiple and changing priorities
Strong analytical and problem-solving skills
Self-directed
Demonstrated problem solving, data assembly and action planning ability leading to the development of actions plans that will prove helpful in achieving measurable objectives

Demonstrative ability to work without supervision towards a desire outcome
Expertise in Microsoft Office and Database management
