I have 5 PDFs that are basically data entry forms, meaning they are printed out and filled out by pen/pencil. Each PDF has about 10 pages but the PDFs share many of the same data entry fields. I want them re-formatted as aesthetically pleasing MS Word docs and fillable PDFs. They need to be aesthetically pleasing and balanced in their choice of font/spacing etc They have sections that need to be re-done as multiple choice, yes/no. Proper spelling and grammar.
