Provides appropriate intervention services and discharge planning to patients with substance use disorders within the Emergency Department of the Stanford Health Care Tri-Valley hospital. Conducts initial brief assessments, introduces patients to substance use treatment programs and services, expedite appointments at Medication Assisted Treatment-capable clinics, serves as the primary coach for their clients, and maintains ongoing contact with their patient panel. Assist with access to other services such as financial counseling, primary care, mental health services, social services, and residential treatment facilities.
