This is an internal agency position; see belowfor the requirements and benefits of this position:

  • Must have flexible schedule - ability to work 36-40 hours eachweek via 8- or 12-hour shifts

Position Summary:

 

Is responsible for providing direct care to residents in a safe manner in accordance with the resident's care plan and accurately record care given. Resident care includes, but is not limited to: Providing and teaching activities of daily living, performing restorative nursing procedures, assessing and reporting resident change to a licensed staff member.

 

Major Responsibilities:

 

Specific:

 

  1. Provides care to residents as planned by the interdisciplinary Care Plan Team and uses safe techniques which are in accordance with the Upjohn Community Care Center and regulatory policies and procedures
  2. Assists the resident to achieve and maintain their highest level of functioning in accordance with the care plan and provide input to the Nursing Supervisor for appropriate care plan changes
  3. Must be able, during a facility emergency/disaster, to report to an assigned area, which will be assigned by their supervisor, and perform the necessary tasks to ensure the calm, orderly and safe handling of residents, staff and public to either move them to safety and /or remove from the threat causing the emergency/disaster
  4. Consistently works cooperatively with the Interdisciplinary Care Plan Team and families to develop, implement and review plans of care
  5. Must use acceptable infection control procedures including Universal Precautions when providing care to residents
  6. Cares for medical equipment and supplies used for residents to insure proper care and charges are made
  7. Reports inoperable, faulty or unsafe conditions, equipment and furniture to Nursing Supervisor
  8. Accurately completes documentation in the medical record to reflect care rendered and reports significant changes in resident condition to the Charge Nurse
  9. Attends training programs and in-services to meet Upjohn Community Care Center and regulatory guidelines
  10. Maintains a positive attitude and contributes to the facility interdepartmental team by completing assignments in a timely, accurate and professional manner
  11. Complies with HCK Customer Service Standards and UCCC- CNA Standards of Care
  12. Follow Resident Rights at all times
  13. Assist new employees in following established UCCC and regulatory policies and procedures
  14. Responsible for assuring resident safety
  15. Other duties as needed or assigned by supervisor

Required Licenses, Registration, Certifications:

 

Must be a certified nursing assistant or graduate of an approved 75 hour class and become certified within four (4) months of hire

 

Knowledge and Critical Skills/Expertise:

  1. Must be able to read, write and comprehend English
  2. Must be able to follow written and oral instructions
  3. Must be able to safely lift, transfer and ambulate residents
  4. Must possess psychosocial skills to be able to interact effectively with residents, staff, families and visitors
  5. Does not pose a direct threat to the health or safety to residents as well as other individuals in the work place

Essential Physical Functions:

 

PHYSICAL ACTIVITY REQUIREMENTS

  1. Lift up to 10 lbs:Required when lifting small objects weighing approximately 5 to 10 lbs
  2. Lift 11 to 25 lbs: Constantly required
  3. Lift 26 to 50 lbs: Constantly required
  4. Lift over 50 lbs:Frequently required transferring a non-ambulatory resident weighing between 110 to 160 pounds
  5. Staff assistance is available when resident is unusually heavy, combative. Transfer belts, low-lifts and Hoyer lifts are available
  6. CARRY up to 10 lbs:Occasionally required to carry small objects from one location in facility to another (50 to 100 feet). If carrying more than one object, may implement the use of a cart
  7. CARRY 11 to 25 lbs:Occasionally required when moving oxygen tanks; emergency drug boxes
  8. CARRY 26 to 50 lbs:Occasionally required when moving oxygen concentrator. Frequently when assisting an ambulatory resident
  9. CARRY over 50 lbs:Occasionally to frequently required when transferring a non-ambulatory resident weighing between 100 to 160 pounds. Two staff persons are typically utilized when resident is unusually heavy or combative
  10. REACH above shoulder height:Frequently occurs while reaching for supplies on upper shelves
  11. REACH at shoulder height:Constantly occurs while documenting in chart, setting up medications, and providing care to residents
  12. REACH below shoulder height:Frequently occurs when reaching for clerical supplies or charting information in drawers or medications in lower parts of med cart
  13. PUSH/PULL:Frequently required to push/pull medication cart, desk drawers; or resident wheelchair.
