Who you are:

We’re looking to build our clinical team with a dynamic and experienced Health Coach. As a Health Coach with knownwell, you will play a pivotal role in supporting our primary care and metabolic health patients on their journey towards healthier lives by providing guidance, education, and motivation. This role is crucial in helping patients make sustainable lifestyle changes, manage their weight, and improve overall health outcomes.

What you'll do:

Patient Engagement:

• Establish strong rapport with patients to gain their trust and facilitate open communication.

Assessment and Education:

• Conduct thorough initial assessments of patients' health, lifestyle, and goals to develop personalized action plans.
• Collaborate with patients to set realistic, achievable health goals and track progress over time.
• Use motivational interviewing and behavior change techniques to help patients overcome obstacles and adopt healthier habits.
• Offer emotional support and encouragement to patients, especially during challenging moments in their health journey.
• Provide evidence-based information on nutrition, physical activity, stress management, and other relevant topics to empower patients to make informed decisions.

Team Collaboration:

• Work closely with the healthcare team, including physicians, dietitians, and nurses, to ensure coordinated and patient-centered care.

Continuing Education:

• Stay updated on the latest developments in obesity medicine, primary care, and health coaching through ongoing training and professional development.

Administration:

• Create care plans, take appropriate notes for billing, and for patients engaged in Remote Patient Monitoring, review such data as well.

What you bring:

• Bachelor's degree in a relevant field (e.g., nutrition, exercise science, psychology, health coaching) required; master’s degree preferred.
• Certified Health Coach (e.g., ACE, NASM, Wellcoaches) or equivalent certification preferred.
• Prior experience as a health coach in obesity medicine or primary care settings is highly desirable.
• Strong interpersonal and communication skills.
• Empathetic and patient-centered approach to care.
• Ability to work effectively within a multidisciplinary healthcare team.
• Demonstrated commitment to continuing education and professional growth.
• Understanding of HIPAA and patient privacy regulations.
