Ohel is seeking a Case Manager to manage the care of adults/ children enrolled in Ohel’s Clinic Programs. The Case manager will provide targeted case management to individuals in the clinic programs, and address social determinants of health through linkage, referral and care coordination. The Case Manager will assess mental health and social services needs and will be responsible for developing an integrated plan of care, working collaboratively with medical, behavioral, educational and social service providers. The case manager will also provide care coordination and health promotion, transitional care and follow up, individual and family support, referrals to community and social support services as well as the use of health information technology to link services.

Position requires an Associate Degree with 2 years case management experience or Bachelor’s degree in the Human Services field. This full time position is based in Brooklyn.
