Piper Clinical Solutions is actively seeking a Remote Case Management Nurse to join a well-established healthcare team that oversees utilization patterns and delivers appropriate health care services for both members and providers. Case Management positions are remote and adhere to aMonday - Friday, traditional hours schedule.

Responsibilities of the Remote RN Case Manager:

• Act as a health coach to members with chronic illnesses by educating them on services, plans of care, post-op transitions, discharge, etc.
• Complete assessments of high-risk patients to determine their level of need and review alternative treatment plans as needed
• Communicate with providers, members, pharmacy and other healthcare personnel to properly meet needs, while remaining cost effective.
• Appropriately monitor pain management related to various chronic conditions of members

Qualifications of the Remote RN Case Manager:

• Active RN in NC or compact licensure
• 3+ years of direct clinical experience in an acute medical or surgical setting
• Previous experience managing care for patients with multiple co-morbidities, preferred
• Previous experience coaching or educating patients regarding care plans, preferred
• Telephonic RN experience, preferred
• Must live in one of the following states: Alabama, Arizona, Arkansas, Colorado, Florida, Georgia, Idaho, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maryland, Michigan, Mississippi, Missouri, North Carolina, Ohio, Oklahoma, Pennsylvania, South Carolina, South Dakota, Tennessee, Texas, Utah, Virginia, Wisconsin, and Wyoming

Compensation of the RN Case Manager:

• Salary: $34-38/hr.
